Introduction
Under DASR 21, the following approval holders are required to monitor in-service usage of their product, change to product, repair design, part or appliance in order to detect deficiencies that have resulted, or could result in an unsafe condition:
- Military Type Certificate (MTC) holders
- Military Supplemental Type Certificate (MSTC) holders
- Major Repair Design Approval holders
- Australian Military Technical Standard Order (AUSMTSO) Authorisation holders
- Military Design Organisation Approval holders
- Military Production Organisation Approval holders
- Manufacturers holding a letter of agreement under DASR 21 Subpart F
DASR AMC GR.40 – Occurrence Reporting contains a non-exhaustive list of occurrences that would require investigation by the relevant design approval holder and are reportable to DASA.
Within an overall limit of 72 hours, the urgency of the occurrence report should be proportional to the hazard arising from the occurrence and may be made with limited information available at the time. The occurrence report (or follow-on reports ) must also contain what action the approval holder has taken, or intends to undertake, in response to the potential safety deficiency. Upon becoming aware of a potential safety deficiency, approval holders should immediately take action to eliminate the risk so far as is reasonably practicable (SFARP), or if the risk cannot be eliminated, minimise the risk SFARP. Approval holders should not wait for DASA to formally respond to an occurrence report before taking action themselves to correct the potentially unsafe condition.

What We Do
DASA will collect occurrence reports and may take action or mandate corrective actions when necessary.
How Do I Report An Occurrence?
All reportable occurrences can be reported directly to the relevant Delegate of the Safety Authority (DoSA) or to the DASA Occurrence Reporting mailbox. An occurrence can be made by any means, including phone, email, Sentinel or DASR Form 44, provided it meets the minimum reporting requirements IAW DASR GR.
